Den startet med kronikken <a href="http://morgenbladet.no/ideer/2015/09/likeverd-pa-samleband">Likeverd på samlebånd</a> av legene Henrik Vogt og Andreas Saxlund Pahle. <a href="http://morgenbladet.no/ideer/2015/09/psykiatriske-behandlingspakker-erfaringer-fra-den-danske-kliniske-dagligdag">Et kritisk innlegg av to danske psykiatere og en psykolog fortalte om erfaringer i Danmark</a>, som er helseministerens forbilde i denne saken. Psykologspesialist og avdelingsleder ved Stangehjelpa, <a href="http://psykologisk.no/2015/01/hva-pasientene-har-lart-meg/">Birgit Valla</a>, hadde innlegget <a href="http://morgenbladet.no/2015/09/det-er-greit-ta-feil">Det er greit å ta feil</a> i Morgenbladet. I går hadde Valla sammen med Scott D. Miller <a href="http://t.co/GoILLqgqVB">en kronikk i Dagens Medisin</a> der de skriver at tjenestene må holdes ansvarlig for resultatene de leverer.<br />

Henrik Vogt har laget <a href="http://henrikvogt.com/2015/09/21/dette-er-en-samleside-for-debatten-om-pakkeforlop-i-psykiatri/">en samleside</a> over innlegg om pakkeforløp i ulike medier.<br />
<br />
Jeg er kjempeskeptisk til pakkeforløp. Pakkeforløpsterapi tar utgangspunkt i hvilken psykiatrisk merkelapp brukeren får. For mitt vedkommende kan det innebære at jeg, som opplevde gjentatte overgrep i over ti år som barn, får like kort terapi som en som får diagnosen PTSD etter en trafikkulykke. <span style="font-family: inherit;">However, the Commissioner points out that the
implementation of the CRPD in Norway falls short of some of </span><span style="font-family: inherit;">its key objectives in promoting the
self-determination, legal capacity and effective equality of people
with </span><span style="font-family: inherit;">psycho-social and intellectual disabilities.
The best interest considerations continue to prevail over the CRPD </span><span style="font-family: inherit;">approach highlighting the </span><span style="font-family: inherit;">persons autonomy, will and preferences. </span><span style="font-family: inherit;">The new Guardianship Act, which entered </span><span style="font-family: inherit;">into force in 2013, still enables substituted
decision-making and plenary guardianship with reference to
psychosocial</span></div>
<div align="left">
<span style="font-family: inherit;">and intellectual disabilities. This is not in
line with CRPD Article 12. The Commissioner is also concerned </span><span style="font-family: inherit;">that the guardianship model hinders the
development of supported decision-making alternatives for those who </span><span style="font-family: inherit;">simply want assistance in making decisions or
communicating them to others.</span></div>
<div align="left">
<span style="font-family: inherit;">The Commissioner urges the Norwegian
authorities to develop new systems for supported decision-making, </span><span style="font-family: inherit;">based on individual consent. Such systems
should be developed together with people with disabilities and </span><span style="font-family: inherit;">along with measures for universal design and
reasonable accommodation. Plenary guardianship and full </span><span style="font-family: inherit;">incapacitation regimes should be revoked as a
matter of priority, and information should be made available on </span><span style="font-family: inherit;">the scope of guardianships applied under the
current system.</span></div>
<div align="left">
<span style="font-family: inherit;">The Commissioner welcomes the Norwegian
national strategy 2012-15 to reduce the use of coercion in mental </span><span style="font-family: inherit;">health care but stresses that a more
comprehensive approach, including legislative reforms, will be required
to </span><span style="font-family: inherit;">bring about fundamental changes. There is a
clear European trend towards reinforcing the rights and
selfdetermination </span><span style="font-family: inherit;">of patients and their participation in
decisions about care, and people with psycho-social </span><span style="font-family: inherit;">disabilities should not be excluded from this
development. All people with disabilities have the right to enjoy </span><span style="font-family: inherit;">the highest attainable standard of health
without discrimination and the care provided to them should be</span></div>
<div align="left">
<span style="font-family: inherit;">based on free and informed consent in line with
Article 25 of the CRPD.</span></div>
<div align="left">
<span style="font-family: inherit;">The Commissioner calls on Norway to reform
legislation on involuntary placements in a way that it applies </span><span style="font-family: inherit;">objective and non-discriminatory criteria which
are not specifically aimed at people with psycho-social </span><span style="font-family: inherit;">disabilities. Precise data on the use of
involuntary medical treatment and restraints in Norway should be made </span><span style="font-family: inherit;">available with a view to drastically reducing
the recourse to such practices. Medical treatment should be based </span><span style="font-family: inherit;">on free and fully informed consent with the
exception of life-threatening emergencies when there is no </span><span style="font-family: inherit;">disagreement about the absence of legal
capacity. The Commissioner is not convinced that the documented </span><span style="font-family: inherit;">involuntary use of ECT in Norway is in line
with human rights standards.</span></div>

<span style="font-family: inherit;">36. While the Commissioner commends Norway for
ratifying the CRPD, he points out that the</span></div>
<div align="left">
<span style="font-family: inherit;">implementation of the Convention in Norway
falls short of some of its key objectives in promoting the </span><span style="font-family: inherit;">self-determination, autonomy, legal capacity
and effective equality of people with psycho-social and </span><span style="font-family: inherit;">intellectual disabilities. The best interest
considerations and substituted decision-making continue to </span><span style="font-family: inherit;">prevail over the CRPD approach highlighting the
</span><span style="font-family: inherit;">persons autonomy, will and preferences. </span><span style="font-family: inherit;">The</span></div>
<div align="left">
<span style="font-family: inherit;">Commissioner urges the government to adopt a
more pro-active stance in implementing its obligations </span><span style="font-family: inherit;">under the CRPD in close cooperation with people
with disabilities and organisations representing them.</span></div>
<div align="left">
<span style="font-family: inherit;">37. In the
Commissioners opinion, the withdrawal of Norways interpretative declarations
concerning the </span><span style="font-family: inherit;">CRPD would signal a new approach. The
Commissioner also encourages Norway to sign and ratify the </span><span style="font-family: inherit;">Optional Protocol to the CRPD on an individual
complaints mechanism which would improve the access </span><span style="font-family: inherit;">of people with disabilities to external review
of their concerns.</span></div>

<span style="font-family: inherit;">38. The Commissioner acknowledges that the
Norwegian authorities have attempted to develop some</span></div>
<div align="left">
<span style="font-family: inherit;">elements of supported decision-making in the
context of the new Guardianship Act and that the number </span><span style="font-family: inherit;">of people put under guardianship with a loss of
legal capacity is relatively small. However, he points out </span><span style="font-family: inherit;">that the new guardianship system continues to
enable substituted decision-making and plenary </span><span style="font-family: inherit;">guardianship and hinders the full development
of supported decision-making alternatives for those who </span><span style="font-family: inherit;">simply want assistance in making decisions or
communicating them to others.</span></div>
<div align="left">
<span style="font-family: inherit;">39. In order to fully comply with the
requirements of Article 12 of the CRPD, the Commissioner urges the </span><span style="font-family: inherit;">Norwegian authorities to develop new systems
for supported decision-making alternatives, based on </span><span style="font-family: inherit;">individual consent. Such systems should be
developed in coordination with measures for universal </span><span style="font-family: inherit;">design and reasonable accommodation, and
together with people with psycho-social and intellectual </span><span style="font-family: inherit;">disabilities. Robust safeguards are needed to
ensure that any support provided respects the will and </span><span style="font-family: inherit;">preferences of the person requesting it and is
free of conflict of interests. Plenary guardianship and full </span><span style="font-family: inherit;">incapacitation regimes should be revoked as a
matter of priority and information should be made </span><span style="font-family: inherit;">available on the scope and specific conditions
of guardianships applied under the current system.</span></div>

<span style="font-family: inherit;">40. The Commissioner welcomes the Norwegian

(Liberty and security of the person), the Commissioner urges the </span><span style="font-family: inherit;">Norwegian authorities to reform legislation on
involuntary placements in a way that it applies objective </span><span style="font-family: inherit;">and non-discriminatory criteria which are not
specifically aimed at people with psycho-social disabilities, </span><span style="font-family: inherit;">while ensuring adequate safeguards against
abuse for the individuals concerned.</span></div>

<span style="font-family: inherit;">42. The Commissioner points out that precise
data on the use of involuntary medical treatment and</span></div>
<div align="left">
<span style="font-family: inherit;">restraints in Norway, including the length of
their application, should be made available with a view to </span><span style="font-family: inherit;">drastically reducing and progressively
eliminating the recourse to such coercive practices. The</span></div>
<div align="left">
<span style="font-family: inherit;">availability of supported decision-making
alternatives and reasonable accommodation measures can</span></div>
<div align="left">
<span style="font-family: inherit;">contribute significantly towards the
development of alternatives to coercion. The Commissioner stresses </span><span style="font-family: inherit;">that medical treatment should be based on free
and fully informed consent with the exception of lifethreatening </span><span style="font-family: inherit;">emergencies when there is no disagreement
regarding absence of legal capacity. The </span><span style="font-family: inherit;">Norwegian Patient and User Rights Act (Chapter
4A) also highlights this principle.</span></div>

<span style="font-family: inherit;">43. It is essential that the use of highly
intrusive treatments such as ECT is subject to robust safeguards. The </span><span style="font-family: inherit;">Commissioner is not convinced that the
documented involuntary use of ECT in Norway with reference to </span><span style="font-family: inherit;">the principle of necessity in the Penal Code
</span><span style="font-family: inherit;">(Article 47) is in line with human rights
standards, </span><span style="font-family: inherit;">including the provisions of the CRPD. The
Commissioner points out that particular care should be taken </span><span style="font-family: inherit;">to ensure that information given by health
professionals about ECT is correct and complete, including </span><span style="font-family: inherit;">information on secondary effects and related
risks, so that patients are able to express their free and </span><span style="font-family: inherit;">fully informed consent to the procedure. It is
also necessary to collect precise data on the use of ECT </span><span style="font-family: inherit;">and make this available to the public. During
the visit, the Norwegian authorities informed the </span><span style="font-family: inherit;">Commissioner of their intention to issue
national guidelines on the use of ECT and the Commissioner </span><span style="font-family: inherit;">welcomes this development.</span></div>

<span style="font-family: inherit;">44. In the
Commissioners opinion, a comprehensive approach towards reducing coercive measures on</span></div>
<div align="left">
<span style="font-family: inherit;">people with psycho-social and intellectual
disabilities needs to cover a range of different settings in</span></div>
<div align="left">
<span style="font-family: inherit;">addition to specialised psychiatric
institutions. The intention of the Norwegian authorities to include</span></div>
<div align="left">
<span style="font-family: inherit;">municipal care staff in the scope of their
future national guidelines on reducing coercion is</span></div>
<div align="left">
<span style="font-family: inherit;">commendable. Cooperation between local and
specialised care providers will be useful in this context.</span></div>

<span style="font-family: inherit;">The Commissioner points out that the use of

Det er noe som er fundamentalt galt i samfunnet vårt når vi har et hjelpeapparat som tillater at mennesker blir brukt som forsøkskaniner i eksperimenter som ikke har noen som helst vitenskapelig evidens, og at man blir degradert fra å være menneske til å bli et objekt som psykiatrien kan gjøre hva den vil med, uten noe som helst strafferettslig ansvar når det virkelig går galt.

psychology was dominated by individual perspectives, Trevarthen was one of the
first researchers to introduce the term intersubjectivity. His analyzes of
mother – baby and father – baby relations showed how babies themselves initiate
a dialogic relationship with adults, and continue this dialogue through eye
contact, smiling, and other bodily movements, rhythmically and cooperatively. He
showed that this interplay is about more than learning by imitation and about
more than developing patterns of attachment. Through his groundbreaking research
he has helped us to understand the dialogical basic quality of human life. He
now says, “In these studies, I have realized that human life is not psychology,
it’s music”.<o:p></o:p><br />

futures</em> in psychotherapy, social work, social sciences, conflict
resolution, and mediation. She is the author of <em>Relational Responsibility:
Resources for Sustainable Dialogue</em>, with Kenneth Gergen. McNamee advocates
a relational ethic based on difference. In one of her recent writings (<em>The
Difference of Dialogue. Toward a relational ethic</em>) she outlines how
dialogue is not about finding common ground or common understanding. Dialogue
evolves from <em>difference</em>, and attention to difference is crucial to the
transformative possibilities of dialogue. Dialogue is about inviting the
unexpected, she says. Her works are inspired by philosophers such as
Wittgenstein, Bakhtin, Searle and Bateson.<o:p></o:p><br />

British anthropologist and Chair of Social Anthropology at the University of
Aberdeen. </span>In his recent work, he links the
themes of environmental perception and skilled practice. Against traditional
models of genetic and cultural transmission, founded upon the alliance of
neo-Darwinian biology and cognitive science, Ingold presents a relational
approach focusing on the growth of embodied skills of perception and action
within social and environmental contexts of human development. Our humanity,
Ingold argues, does not come ready-made, but is continually fashioned in our
movements along ways of life. Starting from the idea of life as a process of
wayfaring, Ingold presents a radically new understanding of movement, knowledge
and description as dimensions not just of being in the world, but of being alive
to what is going on there. Ingold argues that anthropology should be a practice
of engaging in the lives of others, with the potential to contribute to real
transformation – as a prospective endeavor. <o:p></o:p><br />

the last eight years (<em>Beyond learning, Good education in an age of
measurement and The beautiful risk of education</em>), Biesta explores ideas
about what it means to be human, and how such ideas influence how we think about
education - and ultimately, how what is at stake in education is humanity
itself. Biesta asks what might happen to the ways in which we educate if we
treat the question "what does it mean to be human?" as a radically open
question; a question that can only be answered by engaging in intersubjective
dialogue, rather than as a question that needs to be answered before we can
educate. Throughout his writings he provides concrete suggestions for engaging
with the question of purpose in education in new, more precise and more
encompassing ways, with explicit attention to the ethical, political and
democratic dimensions of education. His recent work focuses increasingly on the
phenomenon of teaching and the experience of being taught.<o:p></o:p><br />

Hauges mektige dikt om håp</a>: «Det er det traumet vi ber på. Som ei morgonstund vil få oss til å glida inn på ein våg me ikkje har visst um.»<o:p></o:p></div>

Utgangspunktet til essayisten er en ordbokdefinisjon: «Traume kjem frå gresk og tyder skade eller sår. I definisjonen heiter det at eit traume er ei overveldande og ubehageleg hending som trekkjer ut i tid slik at personen blir prega av hendinga.»<o:p></o:p></div>

Eventyrforskeren Marina Warner fikk nylig den prestisjetunge <a href="http://www.holbergprisen.no/" target="_blank">Holbergprisen</a>, ansett som en «Nobelpris» innen humanistisk forskning, <a href="http://www.aftenposten.no/kultur/Holbergprisen-til-britisk-eventyrforsker-7934584.html" target="_blank">ifølge Aftenposten</a>. I et intervju i<a href="http://morgenbladet.no/kultur/2015/epler_og_gull"> Morgenbladet</a> sier hun at det finnes en gjenkjennelig eventyrverden, et landskap, en særskilt forestillingsverden, også i film, skuespill, reklame og mote.<br />

Det i dette landskapet at essayisten synes å ha realitetsorientert seg:</div>

Og så står vi der då, blenda av lyset og snipp snapp snute.<o:p></o:p></div>

Ifølge Marina Warner er det magiske et element i eventyrenes verden, og traumeoptimismen har mange trylleformularer. «Der det er vilje, er det vei,» sier man, så positivt og snilt som bare det, uten å vite hvor hardt det kan ramme mennesker som har brukt opp all viljestyrke for lenge siden.<o:p></o:p></div>

Jeg er ikke en av essayistens «vi», og hennes tunnel hører ikke meg til. Min metafor er et landskap med lys og skygger og en blindsone som er skapt av samfunnets kollektive mangel på frihet til å se og høre det som er. Verden trenger nye eventyr som kan gjøre blindsonen synlig.<br />

Fra mitt ståsted er det tabu å merke omgivelsenes diffuse forbud mot å se og høre det som er, og senskader som skyldes ekstrem avmakt og isolasjon er også tabu.</div>

«Du må være takknemlig for at erfaringene dine har gjort deg til et varmt og klokt menneske,» svarte en prest straks da jeg sa at jeg var blitt utsatt for presteovergrep som barn. Han hadde nok vært enig med essayisten. Jeg er mer på linje med den irske presten som sa at folk ikke blir snille av å være fattige. «Av fattigdom blir man bare fattig.»</div>

Det går an å være snill selv om man er fattig. Det går an å vokse trass i motgang. Snillhet og vekst viser ikke hvor nyttig det er med fattigdom og traumer. Snillhet og vekst viser hvor sterke og ukuelige mennesker er.<o:p></o:p><br />

I 1950-årene lærte jeg at jeg syndet i tanker, ord og gjerninger, at fortvilelse var en dødssynd, at Gud alltid så meg og ville at jeg skulle «være et solskinnsbarn og lyse for ham hver dag». Og jeg lærte at Gud aldri ga meg større byrder enn jeg kunne bære. Jeg lyste så godt jeg kunne, jeg. Prøvde å være snill. Strevde verre med å unngå dødssynden fortvilelse og syndige tanker som «jeg er lei meg».<br />

Selv om den er bunnsolid, velskrevet og spennende, selv om jeg var forberedt og trodde jeg visste hva jeg ville møte. Hardt fordi den tvinger meg til å ta inn over meg lidelse, svik, smerte og urettferdighet i betydelige doser. Og hardt fordi den røsker så ubønnhørlig i tenkningen min, den viser at legespråket, det faglige begrepsapparat, kategoriene og merkelappene jeg tenker med er beheftet med feil, og derfor gjør det vanskelig å fatte hva visse typer helseproblemer handler om.<o:p></o:p></div>

Hardt også fordi jeg fornemmer at Anna Luise Kirkengen er en ensom krigsreporter som skriver sine blodige og anklagende korrespondentbrev fra en krig som mange leger - og derfor mange andre - vil benekte eksistensen av. Eller bagatellisere som en minikatastrofe blant så mye annet vondt i verden, ikke stort å bry seg om, <b>det kan da vel umulig være den krigen som skaper så mange av disse sårede, redde, skam- og smertefulle menneskene som ber om hjelp vi ikke har, som ruser seg, som blir kronisk syke, som ikke klarer å jobbe, som dør unge?<o:p></o:p></b></div>

Ofrenes ekstreme sårbarhet oppfattes av verden rundt som en dårlig egenskap. <b>Menneskene blir problemet, ikke historien deres - som ofte ligger godt gjemt også for dem det gjelder.</b></span><b><o:p></o:p></b></div>

Grader av avmakt kan ikke tallfestes, men jeg ser for meg en kurve som begynner med spedbarnets totale hjelpeløshet, synker ned til en trygg barndoms små tap av retten til «nei» – og stiger derfra. Uansett hvor et menneskes erfaringer er på denne kurven, er det mulig å ta utgangspunkt i dem, åpne for egen blindsone, tilegne seg kunnskaper, og, ikke minst, innse hva man ikke vet om en annens liv, hva man ikke har mulighet til å vite.<o:p></o:p><br />

Dessverre er traumeoptimisme et viktig element i samfunnets kollektive eventyr. Når noen har det vondt, er det sosialt akseptabelt å lesse på med dypiteter som at andre har det verre og at himmelen bak skyene er blå. <o:p></o:p></div>

Legen Rebekka Helland, som har vært i en alvorlig skredulykke, sier i Aftenposten <a href="file:///D:/TRAUMEOPTIMISME%2025.docx#_edn13" name="_ednref13" title=""><span class="MsoEndnoteReference"><span class="MsoEndnoteReference"><span style="font-family: Georgia, serif;"><span style="color: black; font-size: xx-small;">[13]</span></span></span></span></a> at hun har lært verdien i å kunne si ting slik de er av og til. Hun forteller om venninnen som spurte hvordan det sto til og som sa, etter et ærlig svar: «Faen, så jævlig!»<o:p></o:p></div>

Tanken på at traumer kan føre til noe godt er en moderne variant av dypiteten «aldri så galt at det ikke er godt for noe». Enhver som vil realitetsorientere seg kan finne mange eksempler på at traumer skaper sårede, redde, skam- og smertefulle mennesker, og det er rikelig med informasjon om hvor grensa går.<o:p></o:p></div>

Det er også slik at mennesker som har vært utsatt for langvarige traumer kan ta utdanning og fungere i jobb i lange perioder, før de ikke lenger greier å holde tilbake reaksjoner på traumene. Elsa Almås skrev om dette i Klassekampen 24/6 2013, med utgangspunkt i sine erfaringer som psykoterapeut,<a href="file:///D:/TRAUMEOPTIMISME%2025.docx#_edn14" name="_ednref14" title=""><span class="MsoEndnoteReference"><span class="MsoEndnoteReference"><span style="font-family: Georgia, serif;"><span style="color: black; font-size: xx-small;">[14]</span></span></span></span></a> og jeg har selv erfart å kunne jobbe og fungere i årevis før kroppen tvang meg til å se nærmere på gamle skader.<br />

Begrepet posttraumatisk vekst er et produkt av den positive psykologien. Han viser til tre faktorer som avgjør hvor godt man klarer seg etter en traumatisk hendelse. De er så viktige at jeg setter dem opp som punktliste:<br />

«Har du eit traume, så har du ein kompetanse,» skriver essayisten og viser til incestsentrene, det brukerstyrte værestedet Stabburshella og Malala som eksempler på posttraumatisk vekst.På et bokstavelig plan er denne dypiteten helt korrekt, men fins det eksempler på at det ikke er sant?</div>

Incestsentrene gjør en viktig innsats. Etter et traume fikk hun diagnosen schizofreni - som de senere igjen mente var feil - og ble tvangs"behandlet" i to år. Hun lå <a href="http://www.dagbladet.no/2013/04/10/nyheter/innenriks/psykisk_helsevern/26512818/">over 200 timer bundet fast til en seng</a>.<br />
